• Conduct thorough assessments of patients' psychological and emotional needs, utilizing trauma-informed approaches to develop personalized treatment plans.
• Provide telehealth services utilizing evidence-based interventions to address mental health concerns, particularly those related to grief, anxiety, depression, and trauma.
• Collaborate with interdisciplinary teams, including doctors, nurses, and other healthcare professionals, to create a cohesive treatment strategy for patients.
• Monitor and evaluate patients' progress, adjusting treatment plans as necessary to support optimal outcomes.
• Document all interactions and clinical assessments clearly and accurately, ensuring that all services provided are justified by medical necessity.
• Actively research and recommend community resources, such as food assistance, healthcare access, and support groups, to enhance the quality of life for patients and their families.
• Manage crisis interventions for individuals and families experiencing emergencies or acute mental health issues, ensuring swift and effective response.
• Conduct follow-up visits or calls to assess changes in patients’ situations and provide continued support through transitions in care.

This role primarily focuses on supporting patients within the hospice care setting, which requires a deep understanding of end-of-life issues, grieving processes, and the psychological impacts of terminal illness on individuals and their families. You will work closely with elderly patients, those suffering from chronic or terminal illnesses, and provide emotional and psychological support to their families during this challenging time.

• A Master's Degree in Social Work (MSW) from an accredited institution.
• A valid Licensed Clinical Social Worker (LCSW) license in Pennsylvania.
• Minimum of 2 years of experience providing mental health services, particularly in healthcare settings related to hospice or palliative care.
• In-depth knowledge of mental health conditions and therapeutic practices, especially concerning grief and trauma.
• Exceptional listening, counseling, and communication skills to effectively connect with patients and their families.

• Experience working with veterans and understanding the unique challenges they may face in hospice care.
• Familiarity with telehealth platforms and electronic health record (EHR) systems for documentation.
• Strong advocacy skills for navigating community resources and support services for patients.
• Demonstrated ability to manage crisis situations effectively and compassionately.

This full-time position offers a competitive salary range of USD 90,000 to 100,000 annually, commensurate with experience.
