Position Summary
Columbus Community Hospital is seeking a compassionate and knowledgeable
Registered Nurse (RN) with experience in
cardiopulmonary disease and secondary prevention strategies to join our Cardiopulmonary Rehabilitation team. This role provides clinical expertise, patient education, and care coordination to support individuals in achieving improved health outcomes through rehabilitation and lifestyle modification.

Essential Responsibilities
- Provide clinical expertise in cardiopulmonary health and evidence-based secondary prevention strategies.
- Assess patient risk factors and develop individualized plans to support therapeutic lifestyle changes.
- Educate and motivate patients regarding exercise, nutrition, risk-factor reduction, and overall wellness.
- Collaborate with the rehabilitation team to coordinate patient care and identify opportunities for program improvement.
- Monitor patient progress and communicate significant changes in condition to the supervising physician.
- Identify psychosocial concerns that may impact rehabilitation outcomes and patient success.
- Utilize behavior change theories and motivational techniques to assist patients in setting and achieving health goals.
- Promote rehabilitation, disease prevention, and long-term health maintenance.
Qualifications
Required
- Current Registered Nurse (RN) licensure.
- Bachelor of Science in Nursing (BSN).
- Experience caring for patients with cardiopulmonary disease.
- Knowledge of secondary prevention strategies and rehabilitation principles.
- Strong communication, assessment, and patient education skills.
- Ability to work collaboratively within a multidisciplinary team.
Preferred
- Experience in cardiac and/or pulmonary rehabilitation.
- Training or experience in health coaching, behavior change, and lifestyle medicine.
