Job Summary:Reporting to the Medical Director, the Nurse Practitioner (NP) or Physician Assistant (PA) will play a crucial role in delivering comprehensive healthcare services in both telehealth and in-person environments. You will manage chronic and acute conditions, oversee patient monitoring through RPM, and collaborate with interdisciplinary teams to provide patient-centered care. This role also supports CCM and CHI initiatives, ensuring ongoing care for chronic disease management and improving community health outcomes.


Position Title: Nurse Practitioner (NP) or Physician Assistant (PA)


Primary Care | Full-Time | Onsite Greeley, Colorado


Key Responsibilities

  • Patient Assessment:Conduct comprehensive assessments of patients' health conditions, including physical examinations, medical histories, and diagnostic test results, seeing 15-20 patients daily.
  • Diagnosis and Treatment:Diagnose and treat common acute and chronic illnesses, injuries, infections, and other medical conditions. Develop treatment plans and prescribe medications as needed.
  • Preventive Care:Provide preventive care and health maintenance services, including vaccinations, health screenings, and lifestyle counseling.
  • Health Education: Educate patients and their families about health conditions, treatment options, medications, and self-care techniques.
  • Care Coordination:Collaborate with theMedical Director,Operations Manager, Care Navigators, and Medical Assistants to streamline patient care.
  • Telehealth:Offer seamless virtual healthcare consultations, referrals, and follow-up care through our telehealth platform.
  • Record Keeping:Maintainaccurateand up-to-date patient records and documentation in compliance with healthcare regulations and standards.
  • Prescription Management:Manage andmonitorpatients' medication regimens, including adjusting dosages and renewing prescriptions as needed.
  • Compliance:Ensure all documentation and patient care follow federal and state regulations, including HIPAA and telehealth guidelines.
  • Community Health:Support CHI initiatives aimed at reducing healthcare disparities in underserved communities.
  • Follow-ups: Ensure that patients receive appropriate follow-up care, either personally or through coordinated care teams.


Compensation

  • $105,000 $130,000 base salary(depending on experience)
  • Performance-based bonus opportunities

Qualifications

  • Licensed Nurse Practitioner (FNP/AGNP) or Physician Assistant
  • Active Colorado license required
  • New York license strongly preferred
  • Board certification required

  • Excellent communication and patient engagement skills
