About The Role
Transform home-based care-from wherever you are. As Circadia's full-time tele visit NP, you'll conduct remote assessments for home health patients, act on predictive health signals in real time, and coach care teams through virtual touchpoints to drive earlier interventions and reduce preventable hospitalizations.
Please note this role is hybrid - you'll spend three days a week seeing patients in their homes and two days a week delivering care via telehealth.
What You'll Do
  • Remote patient care: Conduct initial and follow-up telehealth assessments on flagged patients; initiate/adjust plans of care; coordinate with referring and primary care providers-all via secure video/audio platform.
  • Data into action: Interpret trend-based insights from the Circadia platform, prioritize high-risk patients, and drive timely interventions to prevent decline and unnecessary ED visits.
  • Close the loop: Complete documentation/charts within the same day.
  • Tele-visit management: Efficiently complete 25+ tele visits per day for designated home health patients in the SoCal/OC area, focusing on new admissions, high-acuity flagged patients, and Circadia alerts.
  • Quality & product feedback: Surface insights to improve virtual workflows and tech integration in a home health setting.
  • Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams.

How Success is Measured (KPIs)
  • Visits: Target 25+ patient tele visits/day
  • Flagged Patients are addressed within 24 hours
  • Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter.
  • Adoption & impact: Demonstrated workflow adherence across patient panels; trend toward fewer falls/hospitalizations and improved home health star ratings over time.

Must-Have Qualifications
  • Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
  • Eligible for Medicare participation; company bills on your behalf.
  • 2+ years NP experience with complex adult populations in home health or post-acute settings; able to practice independently.
  • Comfortable conducting telehealth visits and navigating virtual care platforms.
  • Thrives in a dynamic, evolving startup environment.
  • English required.

$140,000 - $160,000 a year
As a full-time Nurse Practitioner, you will be employed by Circadia Health, Inc. The anticipated annual base salary range for this full-time position is $140,000 - $160,000. The base range is determined by role and level, and placement within the range will depend on a number of job-related factors, including but not limited to your skills, qualifications, experience, and location.
