COMPANY DESCRIPTION: 

Union Rescue Mission (URM) is a 501(c)(3) nonprofit organization located in the Los Angeles Metropolitan Area. We are dedicated to serving men, women, and children experiencing homelessness and providing comprehensive emergency and long-term services to help them escape the dangerous streets of Skid Row. Established in 1891, URM is now one of the largest rescue missions of its kind in the United States and the oldest in Los Angeles. POSITION SUMMARY: 

The Case Manager will develop and facilitate the essential case plan components that provide our guests with the resources to attain the goal of life transformation and self-sufficiency. 

CORE WORK PRINCIPAL: 

Colossians 3:23 is the foundation of URM's standard for employment, "Whatever you do, work at it with all your heart, as though you were working for the Lord and not for people". The goal for each employee is to enrich and sustain a humble heart attitude; to enrich and sustain a mindset of serving others, treating them with kindness and truth; and to enrich and sustain a zealous desire for achieving work objectives without any ulterior motives or agendas.


ESSENTIAL FUNCTIONS:

  • Develops, maintains, and tracks individualized case management plans that outline services, objectives, and outcomes for assigned caseload.

  • Performs case management activities, including but not limited to:

                     -Intake assessments and evaluations to identify services and needs.

                     -Coordinates emergency, legal, medical, transportation, mental health, and other internal support services 

                      for  guests as needed.

                     -Establish program goals and priorities

                     -Provides ongoing case management support to guests through in-person meetings, phone calls, home 

                      visits, written correspondence, and  virtual communication.

                     -Refer residents to essential services such as housing, county benefits, education, and other external 

                      services, as needed.

                     -May facilitate program classes and trainings.

                     -Coordinating plan implementations with other staff.

  • Intervenes and de-escalates volatile and hostile situations on the premises, coordinating with security and emergency personnel as needed.
  • Conduct random drug testing and room inspections, as needed.
  • Maintains client records and submit weekly/ monthly-related case management documentation.
  • Collaborates with management to develop, track, and report program and guests KPI's to align with department goals.
  • Prepares and process formal organizational and guest correspondence and documentation, including sensitive, confidential, official, and legal communications.
  • Assist with volunteer supervision.
  • May travel as required to support training, client services, and program related needs.
  • Conduct other tasks and projects assigned by direct supervisor.
  • Commitment to URM mission, vision, and core values.
  • Encourage guests in their faith and growth in Jesus Christ.

  • Required manual dexterity for occasional reaching and lifting small objects, and operating office equipment.
  • Ability to lift up to 30 lbs. 
  • May travel up to 30% of the time, as needed.

  • Noise levels are considered low to moderate.

Requirements

EXPERIENCE, EDUCATION AND LICENSURES:

  • Must possess the following:

              -Bachelor's degree in behavioral sciences, social work, or related field with a minimum of 2 years 

               of related experience. OR

              -High School Diploma or GED with a minimum of 3 years of related experience.

  • Prior experience working with or serving homeless families preferred.
  • A valid driver's license is required.

KNOWLEDGE, SKILLS AND ABILITIES:

  • Must have excellent time management skills and ability to multi-task in a high stress environment.
  • Ability to communicate effectively in both orally and in writing.
  • Proficient in Microsoft programs including Word, Excel and Outlook and the ability to learn proprietary computer programs.
  • Fluency in Spanish, both verbal and written, a plus.
