Job description

Virtual Nurse

This position provides general nursing care to patients and families along the health illness continuum in diverse health care settings while collaborating with the health care team. He/She is accountable for the practice of nursing as defined by the Ohio Board of Nursing.

Virtual Nurse Responsibilities

The Virtual Nurse will be practicing at the top of their scope working remotely from a centralized location. The Virtual Nurse responsibilities will be centered around care coordination which will include plan of care, evaluation, teaching, guiding teams and coaching nurses. The VN will also do tasks that may include admissions, discharges, assessments and hourly rounds.

The Virtual Nurse responsibilities will include the following:

  • Leads team members in problem solving patient care situations
  • Prioritizing activities flexibly according to changing situations and coaches the team to the plan
  • Views patient comprehensively, including the anticipation of discharge needs
  • Consults others on the interdisciplinary team as appropriate
  • Uses teach-back to ensure the patient's understanding of care
  • Engages the patient in developing, and evaluating the plan of care each day
  • Works to include family in patient care decisions as appropriate
  • Recognizes family dynamics that impact patient care and mitigates barriers which may include arranging interdisciplinary care meetings with family and patient
  • Design, apply and update patient care programs
  • Coordinates intervention of the interprofessional team which may include for transfer, discharge or ensuring the care team understands the goals the patient needs to meet and how each team member contributes
  • Involved with quality audits, implementation of new initiatives on the unit by training others, or evaluating the impact of practice change
  • Stays current with new knowledge and brings new information related to unit priorities to other team members on the unit
  • Able to give and receive constructive feedback and coach others in this skill
Responsibilities And Duties

Assessment/Diagnosis - Performs initial, ongoing, and functional health status assessment as applicable to the population and or individual (30%).

Outcomes Identification/Planning - Based on nursing diagnoses and collaborative problems, documents planned nursing interventions to achieve outcomes appropriate to patient needs (30%).

Implementation/Evaluation - Evaluates and documents response to nursing interventions and achievement of outcomes at appropriately determined intervals; as part of a multidisciplinary team, revises plan of care based on evaluative data (20%).

Leadership - Actively participates in process improvement activities to achieve targeted measures of clinical quality, customer satisfaction, and financial performance (10%).

Operations (10%).
