The RN Discharge Planner is responsible for coordinating daily clinical care and managing the discharge planning process for CalAIM-eligible individuals. This role provides clinical oversight of reentry care plans, ensuring they are appropriate, compliant, and aligned with Enhanced Care Management and Community Supports. The position works closely with managed care plans, community providers, and facility leadership to coordinate care and support successful transitions. The RN Discharge Planner conducts assessments as needed and ensures adherence to applicable nursing standards and regulatory requirements. This role serves as the accountable clinician for the discharge planning process.

Key Responsibilities
  • Provides clinical review and oversight of discharge planning activities, including off‑site, outpatient, and acute care services
  • Reviews and manages utilization, pre‑certification, and documentation within care management systems to ensure accuracy and compliance
  • Coordinates care with medical directors, providers, managed care plans, and community partners to support effective transitions
  • Reviews referrals, claims, and medical records to ensure appropriate level of care, coding accuracy, and cost management
  • Communicates discharge needs and collaborates with facility and operations leadership to determine appropriate placement and release planning
Qualifications & Requirements

Education

  • Unrestricted, current RN license in state of employment
  • BSN, CCM preferred.

Experience

  • Two (2) years’ clinical experience with Discharge Coordination experience
  • Previous correctional/detention facility experience

Licenses/Certifications

  • Must maintain all certifications, educational requirements, licensing, etc. for RN license

We’re looking for candidates with strong RN case management, discharge planning, or care coordination experience, ideally with exposure to CalAIM, Medi‑Cal, managed care plans, Enhanced Care Management, Community Supports, or reentry services. The ideal candidate is comfortable coordinating complex discharge needs, connecting patients to community resources, and helping vulnerable populations transition successfully back into the community. A strong fit would be an RN who is organized, resourceful, patient‑centered, and persistent with follow‑up. This role requires someone who can think beyond the immediate medical need and help create a realistic plan for what happens after the patient leaves the Merced County Jail.
