Job description

Registered Nurse

The RN is responsible for administering the care plan and the patient/client's health and wellness as indicated by the plan of care. Assures compliance with nursing/clinical standards federal, state, and local government regulations, and company policies. This role reports to the Director of Nursing (DON).

Essential Functions:

  • Follows the appropriate Care Plan in collaboration with the Director of Nursing/Case Managers.
  • Participates in the admission, discharge, and re-assessment of Med clients.
  • Obtain medication orders and medication lists directly of client's prescribing physician and/or approved facility per agency policy.
  • Responsible for ensuring that ample medications are available in the home and provide medication education as needed.
  • May be required to assist clients with obtaining re-fills, pharmacy pick up, med lock box management, and Narcotic logs.
  • Provides on-going instructions to agency caregivers, including reinforcement of adhering to the plan of care, Standard Precautions, agency policies, client safety, and HIPPA.
  • Documents services rendered and changes in client conditions and/or family and home situation.
  • Collaboration with other agency disciplines and/or external agencies (i.e. Hospice).
  • Communicates with Case Managers, DON, or client's legal representative/family and concerns or issues regarding client status as necessary.
  • Perform other duties as assigned.

Knowledge, Skills, and Abilities:

  • Excellent problem-solving ability, organizational skills and requires minimal direction.
  • Demonstrated competence in applying processes designated to ensure adherence to all pertinent federal and state rules and regulations.
  • Strong communication skills. Possess the ability to clearly and effectively communicate with staff, physicians, referral sources, patients and families.
  • Demonstrate computer literacy skills.

Qualifications:

  • Graduate of an approved school of professional nursing. Current licensed RN or LPN in practicing state.
  • One or more years of experience and responsibilities in clinical home health services, or equivalent preferred.
  • Must be a licensed driver in the appropriate state and have available independent transportation with current care insurance coverage.
  • Current CPR certification.

Working Conditions:

  • Significant portion of daily assignment involves application of manual skill requiring motor coordination and finger dexterity. Work does involve occupations risk (i.e. needlestick). Routine exposure to infectious hazards, including blood borne pathogens.

Pay Range: This position is compensated on a per-visit basis, with rates of $65.00 for supervisory and 60-day visits and $80.00 for admission visits. Actual compensation will be determined based on a variety of factors, including, but not limited to, the candidate's relevant experience, qualifications, certifications, skills, and geographic location.
