This position is primarily onsite in a home care setting, allowing for close interaction with patients in their daily environments. • Conduct thorough evaluations and create tailored treatment plans to address the unique needs of each patient, based on physician referrals.
• Implement diverse therapeutic modalities and techniques to optimize recovery, focusing primarily on orthopedic, pediatric, neurologic, and sports medicine specialties.
• Monitor and document patient progress meticulously, adjusting treatment strategies as necessary to align with recovery goals and physician's guidance.
• Provide ongoing education for patients and their caregivers to ensure clarity in treatment plans and bolster compliance.
• Maintain a high standard of customer service, addressing concerns promptly and with empathy to enhance patient satisfaction.
• Collaborate with a multidisciplinary team to share insights and strategies that promote effective outcomes across the continuum of care.
• Achieve performance metrics and business objectives that drive both personal and organizational success.

In your role at Viera Physical Therapy, you will work with a varied patient demographic, each facing different physical challenges. From children requiring rehabilitation from sports injuries to elderly patients recovering from surgery, your ability to adapt and apply your skills across specialties will be critical. • A Doctorate in Physical Therapy (DPT) from an accredited educational institution.
• Active State License as a Doctor of Physical Therapy (DPT) issued by the Physical Therapy Examining Committee.
• A minimum of two (2) years of clinical experience in a relevant setting.
• Current CPR certification to ensure patient safety.
• Strong written and verbal communication skills, along with proficiency in computer and documentation software.
• Commitment to building strong relationships with patients to effectively support their rehabilitation goals.
• A genuine enthusiasm for physical therapy and connecting with individuals from diverse backgrounds.

Experience in home health settings, particularly with patients suffering from chronic or complex conditions, is highly regarded. Familiarity with electronic health records (EHR) systems and knowledge of current research and advancements in physical therapy techniques will be beneficial.

The salary for this position ranges from $100,000 to $120,000 annually, commensurate with experience and qualifications.
