Join Our Team at Valley-Wide Health Systems, Inc.
Valley-Wide Health Systems is seeking a compassionate and collaborative Behavioral Health Provider to deliver integrated, whole-person behavioral health care within our team-based primary care model, supporting patients' mental, emotional, and social well-being across the lifespan.
Key Responsibilities
  • Serve as a Behavioral Health Provider (BHP) within a team-based care (TBC) model, delivering whole-person (biopsychosocial) care.
  • Identify and assess at-risk patients, including crisis evaluation and intervention.
  • Conduct diagnostic mental health assessments and screenings; determine level of care and refer to specialty services as needed.
  • Develop and implement biopsychosocial treatment and relapse-prevention plans in collaboration with patients, families, and care teams.
  • Provide evidence-based behavioral health interventions and psychotherapy.
  • Monitor patient progress, ensure timely and accurate documentation, and communicate updates with the interdisciplinary care team.
  • Collaborate with internal and external partners, including Enabling Services, to address social determinants of health.
  • Participate in integrated care meetings and support the ongoing development of integrated behavioral health infrastructure.
  • Maintain professional relationships with community agencies, healthcare partners, and local organizations.

Requirements
  • Master's or Doctoral degree in psychology, counseling, or clinical social work.
  • Licensed or license-eligible in Colorado as a Behavioral Health Provider (LCSW, LPC, LMFT, PsyD, or PhD).
  • LPCCs must complete the Jurisprudence Exam and obtain LPC licensure within three years of hire.
  • Two years of clinical experience preferred.
  • Knowledge of mental health assessment, psychotherapy, crisis intervention, and DSM-5/ICD-10.
  • Ability to pass a behavioral health background check.
  • Valid Colorado driver's license with ability to travel.
  • BLS certification provided upon hire.
