JOB TITLE: Home Health RN Case Manager & Recertification Nurse (RNCM)This position will be hired as PRN or Part-Time only. Additional Part-Time ihours are also available with a 2-hour travel radius to "Secondary" locations listed below.Full-Time may be available in the future but not at this time and is not guaranteed.This is a regional leadership role within our agency, requiring travel as needed throughout Dayton and Metro-Dayton areas where clients and caregivers reside.This role works closely with and reports directly to our SW OH Regional Program Administrator (local) and/or Director of Clinical Administration & Operations (corporate). For those pursuing career growth: a defined path for advancement to Assistant Program Administrator, Program Administrator, Clinical Administrator, Director of Nursing (DON or Asst. DON), Business Administrator, QA / Compliance leadership, and other positions will be supported.Work consists of: case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services, as well as new hire orientations, in-service and supervisory reviews, and input to influence our training and education, care practices and outcomes, and the success of our clients, employees and business overall.
