Members are incredibly fortunate when someone with your keen professional judgment, passion for your work, and strong persuasive power is in charge of their care. In this role, you will independently perform home visits to conduct functional, psychosocial and environmental safety assessments, determine eligibility of enrollees, and establish an individual plan of care that enables them to remain safely at home despite long-term care needs. Your talents for listening and communicating with a culturally diverse population that includes care givers as well as other providers along with your time management skills will be key to your success.
Multiple openings for the following regions: Westchester, Queens (Mandarin or Cantonese speaking preferred ), and Manhattan (Spanish speaking preferred ). We also have openings in our upstate counties such as Dutchess as our primary and Rockland, Putnam, Sullivan, Ulster, Orange, as secondary.
MJHS rewards full-time Assessment Nurses with a higher reimbursement rate when conducting extra visits.
Qualifications:
  • Current NYS RN license and registration
  • Associates degree required; Bachelor of Nursing degree preferred
  • Bilingual BSN new graduates encouraged to apply
  • One year of experience in Homecare, acute care or community health nursing preferred.
  • Working knowledge of Medicare, Medicaid and NYS DOH standards and regulations
  • Bilingual Spanish, Mandarin, Cantonese, Korean, Creole, Bengali, Hindi, and Arabic desirable
