Day Shift: 7:00 AM – 7:00 PM
Night Shift: 7:00 PM – 7:00 AM
Schedule: 3.5 days per week | Every other weekend required

About the Role

Plum Creek Chicago is seeking compassionate, dependable, and resident-focused Certified Nursing Assistants (CNAs) to join our team.

As a CNA, you will provide hands-on care and daily support to residents while promoting dignity, independence, comfort, and quality of life. You will work closely with our nursing team to ensure each resident receives attentive, individualized care in a safe and supportive environment.

We are looking for team members who genuinely enjoy caring for others and want to make a meaningful difference in the lives of our residents each day.

Key Responsibilities
  • Assist residents with activities of daily living, including bathing, dressing, grooming, toileting, and personal care

  • Assist residents with mobility, transfers, positioning, and ambulation as needed

  • Take and record vital signs and report changes or concerns to the nursing team

  • Respond promptly to resident call lights and requests for assistance

  • Assist residents during mealtimes and with feeding as needed

  • Observe residents and communicate changes in condition, behavior, or needs to the appropriate nursing team member

  • Provide companionship, reassurance, and emotional support to residents

  • Maintain accurate and timely documentation of resident care

  • Help maintain clean, safe, and comfortable resident rooms and common areas

  • Follow established infection control, safety, and resident care procedures

  • Work collaboratively with nurses, caregivers, and other team members to provide exceptional resident care

Qualifications
  • Current Certified Nursing Assistant (CNA) certification in the State of Illinois

  • High school diploma or equivalent preferred

  • Strong communication and interpersonal skills

  • Ability to work independently while contributing to a team environment

  • Dependable, compassionate, and professional approach to resident care

  • Basic computer and documentation skills

Preferred Experience
  • Previous experience in long-term care, skilled nursing, assisted living, supportive living, or senior care

  • Experience working with older adults or individuals requiring assistance with daily living
