Registered Nurse (RN) - Avenue at Strongsville

Join our dynamic team at The Avenue at Strongsville, a post-hospital rehabilitation and skilled nursing care facility, as a Registered Nurse (RN). Our commitment to delivering exceptional care and service sets us apart from traditional care settings.

About the Role:

As a Registered Nurse (RN) at The Avenue at Strongsville, you will have the opportunity to make a meaningful difference in the lives of our residents and their families. You will be responsible for providing direct patient care, assessing patient needs, developing and implementing individualized care plans, and collaborating with our interdisciplinary team. With a patient-centered approach, you will help our residents achieve their maximum potential and maintain their independence.

Responsibilities:
  • Provide high-quality patient care in accordance with our facility's standards and regulations
  • Conduct initial and ongoing assessments of patient needs, identify risks, and develop effective care plans
  • Implement and evaluate the effectiveness of care plans, making adjustments as necessary
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Monitor and report changes in patient condition to the medical director and other relevant personnel
  • Maintain accurate and detailed records of patient care and treatment
