Home Health Registered Nurse

All positions are located in Fresno/Clovis CA

You will join a committed team of over 100 Home Health professionals with a fervor for doing good work in our community by initiating, intervening, and evaluating nursing services to meet patients where they are within a 60 mile radius in the surrounding Fresno area. Community is expanding its footprint and that means bringing on more staff to cover our Home Health Department.

Responsibilities

As a Home Health Registered Nurse, you will have the opportunity to collaborate with a team of Home Health professionals with a passion for caring for Valley patients in the safest place to them, their home. Averaging 35-30 visits per week, you'll have the unique opportunity to focus on the continuum of care for a wide acuity of patients, including establishing a plan of care. This is the perfect opportunity if you are looking to grow your nursing career in a health system that prides itself on learning and growth.

Qualifications

Education & Experience

  • Graduation from a School of Nursing or successful completion of sufficient units in Nursing to qualify for state licensure required
  • 1 year of acute care experience required

Licenses and Certifications

  • RN - Current State of California Registered Nurse license required
  • BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
  • DL - Valid California Driver's License and driving record acceptable to Community Health System required
