*All positions are located in Fresno/Clovis CA*

Are you looking for an unique RN position? You met your match. As an RN with Home Health the average day is anything but average and you will actively feel and see how your role impacts our mission and those we serve. You will join a committed team of over 100 Home Health professionals with a fervor for doing good work in our community by initiating, intervening, and evaluating nursing services to meet patients where they are within a 60 mile radius in the surrounding Fresno area. Community is expanding its footprint and that means bringing on more staff to cover our Home Health Department.

Responsibilities

As a Home Health Registered Nurse, you will have the opportunity to collaborate with a team of  Home Health professionals with a passion for caring for Valley patients in the safest place to them, their home. Averaging 35-30 visits per week, you'll have the unique opportunity to focus on the continuum of care for a wide acuity of patients, including establishing a plan of care. This is the perfect opportunity if you are looking to grow your nursing career in a health system that prides itself on learning and growth.

Qualifications

Education & Experience

  • Graduation from a School of Nursing or successful completion of sufficient units in Nursing to qualify for state licensure required
  • 1 year of acute care experience required

Licenses and Certifications

  • RN - Current State of California Registered Nurse license required
  • BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
  • DL - Valid California Driver's License and driving record acceptable to Community Health System required
