District 2 Public Health serves 13 counties in northeast Georgia, including Banks, Dawson, Forsyth, Franklin, Habersham, Hall, Hart, Lumpkin, Rabun, Stephens, Towns, Union, and White counties. Job Responsibilities PH RN 1: Under supervision, provides professional nursing care in accordance with nurse protocols, policies and/or procedures PH RN 2: Under general supervision, provides professional nursing care in accordance with nurse protocols, policies and/or procedures. Focuses on the prevention of illness, injury or disability, the promotion of health and maintenance of the health of populations. Cares for population with special needs during disasters/emergencies and biological and chemical protocols.

Additional Duties: Provide outreach, education, and case management to high-risk perinatal & postpartum women, and infants up to one year of age through home visits. Conduct home visits for prenatal, postpartum, and infant assessments to assess patient conditions and coordinate with providers to ensure appropriate care. Educate patients, offer referrals for additional services, and document observations and follow up plans.

Attend required meetings (local, District, and State). May supervise additional staff as needed. Willing to travel within District 2 as necessary.

Familiar with OB/GYN hospital practices Other duties as assigned Minimum Qualifications PH RN 1: Associate's degree in nursing from an accredited college or university AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification. Note: No experience required. PH RN 2: Bachelor's degree in Nursing from an accredited college or university AND Current Georgia license to practice as a Registered Professional Nurse AND CPR certification OR Associate's degree/diploma in Nursing from an accredited college AND One year of PHN experience AND Completion of a PHN course within one year of hire AND Current Georgia Registered Professional Nurse license AND CPR certification.
