Physical Therapist - Rehab

Location: Elkhart, Indiana

Start Date: 08-05-2026

End Date: 11-04-2026

Contract Type: Travel Contract

Shift Type: Monday - Friday, 8:00 AM - 4:00 PM (Day shift, 40 hours/week)

Hours per Week: 40

Candidate Pay Rate: $49 per hour

Roles & Responsibilities
  • Screen, evaluate, and treat patients to restore function and mobility
  • Develop and update individualized treatment and discharge plans of care
  • Document services accurately and timely to support continuity of care
  • Supervise Physical Therapist Assistants and Rehab Techs per state practice acts
  • Collaborate with interdisciplinary care teams and families to optimize patient outcomes
  • Maintain compliance with safety, infection control, and regulatory standards
Qualifications
  • Graduate of an APTA-accredited Physical Therapy program (DPT, MSPT, or BSPT)
  • Active Indiana state PT license (or ability to obtain prior to start)
  • Strong communication, documentation, and clinical skills
  • Flexibility to adapt to varying facilities, schedules, and patient needs
Education
  • Graduate of an APTA-accredited Physical Therapy program (DPT, MSPT, or BSPT)
Knowledge & Skills
  • Physical therapy evaluation and treatment planning
