Description:

The UAS Registered Nurse is responsible for ensuring that ALP residents receive appropriate care in accordance with their individualized care plans, physician orders, facility policies, and applicable regulations. This position is responsible for conducting resident assessments, maintaining compliance with ALP requirements, supervising HHAs, monitoring resident care needs, and maintaining accurate documentation.

Essential Duties and Responsibilities

  • Conduct initial assessments to determine ALP eligibility.
  • Complete UAS assessments and reassessments as required by New York State regulations and facility policy.
  • Update resident care plans every six months and as needed based on changes in condition or care requirements.
  • Complete Summary Assessments upon admission, 45 days after admission, every six months thereafter, and as needed.
  • Conduct reassessments for ALP residents following hospitalizations or significant changes in condition.
  • Create and maintain ALP resident charts and ensure ongoing compliance with all documentation requirements.
  • Monitor and oversee the implementation of resident care plans by HHAs and other care staff.
  • Review nurse notes daily and follow up on any concerns, changes in condition, or care issues.
  • Verify and confirm services documented on ALP care sheets.

Requirements:
  • Current New York State Registered Nurse (RN) License in good standing.
  • Minimum one year of nursing experience preferred.
  • Experience with UAS-NY assessments preferred.
  • Current documentation of immunity to Measles, Mumps, and Rubella (MMR).
  • Current physical examination completed within 30 days of employment.
  • Current negative PPD test or chest X-ray, as required.
  • Compliance with all facility vaccination requirements.
  • Ability to meet all state and facility requirements for employment.
