About the Role:
We are seeking a highly skilled Physical Therapist to join our team as a PT for Home Health. In this role, you will be responsible for providing exceptional care to patients in their homes, helping them to regain their strength and independence. You will work closely with physicians, nurses, and other healthcare professionals to develop and implement individualized treatment plans that meet the unique needs of each patient. Your ultimate goal will be to help patients achieve their maximum potential and improve their quality of life.

Minimum Qualifications:
  • Current state license as a Physical Therapist
  • A minimum of one year of experience as a Physical Therapist
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and critical thinking skills
Responsibilities:
  • Conducting patient evaluations and developing individualized treatment plans
  • Providing hands-on therapy to patients in their homes
