KidsPeace is a private charity dedicated to supporting the behavioral and mental health needs of children, families, and communities. Since 1882, we have offered psychiatric hospital services, residential treatment programs, accredited educational services, and foster care programs. Our team works in an environment defined by compassion, creativity, collaboration, and a commitment to delivering hope and healing.

Position Overview

This position is responsible for supporting the continuity of care for children by facilitating the exchange of health information, treatment updates, discharge planning, and documentation among internal and external providers. This role serves as a key liaison between residential, medical, educational, and community-based service providers to ensure coordinated and comprehensive care.

Responsibilities include coordinating Medicaid-funded services received in the community, supporting successful transitions to home and community-based living, and assisting with outpatient treatment planning as part of the discharge process. The position facilitates communication and documentation related to medical, behavioral health, and other community services, including participation in community-based care meetings and appointments as needed.

The Care Coordination Specialist is also responsible for arranging and providing medical escorts, transportation, and supervision during healthcare appointments in the community. When a child attends a community school, the role coordinates care and services with educational providers to ensure continuity of treatment and support.

Key Responsibilities

  • Coordinate Medicaid services received by the child in the community and transition the child to home and community-based living with outpatient treatment as part of discharge planning. 
  • Facilitate the exchange of health information and documentation of care received by the child in the community, including attending any community provided care. 
  • Must coordinate care if child attends community school and delivers services through the provider. 
  • All other duties as assigned by management. 

Qualifications

  • Must be at least 21 years of age. 
  • High School Diploma or equivalent required. 
  • Maintain a valid New York State driver’s license. 
  • Certification in 1st Aid/ CPR prior to or after hire.
