The Intertwined Trauma-informed Therapy Clinic is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in a full-time capacity. This remote role provides an opportunity to work closely with marginalized and vulnerable populations, particularly within a hospice center, where our efforts are focused on improving the psychological well-being and quality of life for patients and their families.

This role will operate within a hospice center setting, emphasizing the importance of providing compassionate care to patients facing end-of-life challenges. While the work is primarily remote, you will engage virtually with patients and families, promoting an inclusive approach where cultural sensitivity and holistic understanding are paramount. You will become part of a multidisciplinary team that consists of psychiatrists, psychologists, nurses, and case managers, all dedicated to coordinating comprehensive and compassionate care.

• Conduct thorough mental health assessments, including psychosocial evaluations, diagnostic interviews, and risk assessments to gauge the mental and emotional state of patients.

• Collaboratively develop tailored treatment plans that address each patient’s unique goals, needs, and cultural context, ensuring a personalized approach to care.

• Deliver evidence-based therapeutic interventions, such as CBT, DBT, and solution-focused brief therapy (SFBT) in various settings, including individual, couple, family, and group therapy.

• Provide crisis intervention services, particularly for patients in acute distress, collaborating with healthcare team members to ensure a continuity of care.

• Maintain meticulous clinical documentation, including progress notes, treatment plans, and discharge summaries, in accordance with clinic policies and regulatory requirements.

• Deliver psychoeducation to patients and families, equipping them with knowledge and tools to navigate their mental health challenges effectively.

• Facilitate multidisciplinary consultations with colleagues, collaborating on comprehensive care strategies for optimal patient outcomes.

• Regularly assess treatment effectiveness through outcome evaluations and adjust treatment plans as necessary to enhance patient progress.

• Stay abreast of current research and best practices in mental health to continuously incorporate the latest evidence into your clinical practice.

• Strictly adhere to ethical guidelines, ensuring patient confidentiality and integrity of information throughout all stages of care.

This role primarily focuses on patients within a hospice care context, involving individuals who are facing terminal diagnoses and their families. You will engage with a demographic that often exhibits significant emotional and psychological distress. Your skills in cultural sensitivity will be vital as you address the diverse backgrounds and experiences of our clients, ensuring that all treatment approaches are respectful and inclusive of their unique stories.

• A Master’s degree in Social Work is required, focusing on clinical practice in mental health.

• An active State license as a Licensed Clinical Social Worker (LCSW) is mandatory for this position.

• Proven ability to work both independently and as part of a collaborative, multidisciplinary team.

• Exceptional interpersonal and communication skills that foster rapport with patients from various cultural and socioeconomic backgrounds.

• Experience in providing mental health services within a hospice or palliative care setting is highly desirable.

• Familiarity with trauma-informed care principles and their application in therapeutic settings.

• Knowledge of additional therapeutic techniques beyond CBT and DBT, such as mindfulness-based therapies, is a plus.

• The position offers an annual salary range of USD $70,000 to $84,000, commensurate with experience and expertise.
