Job Description: Transition Specialist
Department: Alabama Department Rehabilitative Services
Reports to: Transition Program Manager
FSLA: Non-Exempt, Full-Time

Job Summary: The Transition Specialist provides job readiness instruction and employment support to high school students and adults with disabilities and other barriers to employment. Responsibilities include teaching job readiness curriculum within local high schools, providing job coaching and support during Summer JET, and facilitating onsite job readiness training for adult participants. This is a full-time position averaging 40 hours per week and includes employee benefits.
Qualifications
  • Bachelor's degree in a related field required.
  • Minimum of two years' experience in vocational rehabilitation, rehabilitation counseling, job coaching, or special education preferred.
  • Strong written, verbal, and interpersonal communication skills.
  • Proven ability to work independently and meet established goals.
  • Demonstrated ability to work effectively with individuals with diverse cognitive abilities and disabilities.
  • Proficient in computer use, including word processing.
  • Must maintain a driving record acceptable to WE Inc.'s insurance provider and carry minimum 100/300 auto insurance coverage.
  • Experience working with young adults or teenagers living with disabilities required.
  • Experience working with individuals from diverse backgrounds preferred.
  • Strong organizational skills.
  • Supervisory experience preferred.
  • Must successfully pass a background check.

Additionally, this individual must have classroom management skills and a general knowledge of disabilities.
Job Responsibilities & Performance Standards:
TRANSITION SERVICES
  • Provide transition services to local students as needed.
    1. Effectively teach the curricula developed by the Transition Program Manager, in accordance with the parameters each school establishes and with the authorizations supplied by referring counselors from the Alabama Department of Rehabilitation Services.
    2. Serve as the liaison for Transition-related programs.
    3. Work with Counselors and ADRS to grow the Transition program and community partnerships.
    4. Assist students with work ethics and work development goals.
    5. Monitor authorizations and request more units as necessary.
    6. Ensure that all participants are entered into approved software for Workshops Empowerment Inc., and document the progress of each student.
    7. Work with administrative staff to ensure that billing for all transition services is completed promptly and accurately.
    8. Document and bill Thrive and classes.
  • Provide prompt and thorough reports.
    1. Following guidelines and provided templates, provide thorough reports on each student to the referring counselor(s).

GENERAL RESPONSIBILITIES
  • Ensure effective communication.
    1. Maintain regular, proactive communication with referring counselors and school personnel to gather feedback and confirm satisfaction with WE Inc.'s services.
    2. Keep all referrals, service, demographic, and termination documentation accurate and up to date.
    3. Protect consumer confidentiality in accordance with WE Inc. policies.
    4. Complete and submit class notes and related documentation within 72 hours.
    5. Collaborate with community partners to ensure business relationship needs are met.
  • Contribute effectively as part of a team committed to advancing the success of the individuals we serve and supporting the organization's mission.
    1. Maintain current knowledge by participating in required training, in-services, conferences, and meetings.
    2. Deliver and accurately document all assigned classes.
    3. Perform additional duties as needed to support program objectives and organizational goals.

Typical Working Conditions: Work is performed primarily in local schools in the Birmingham metro area and adjacent counties. Work may require standing for extended periods of time in a classroom setting, and may require standing, stooping, reaching, job coaching and lifting 40 pounds. Requires manual dexterity sufficient to operate a computer, telephone, copier, and other office equipment, as necessary. Requires normal range of hearing. Requires automotive insurance, an active driver's license, and access to an automobile. Involves contact with staff, school administrators, students, and the public.
Work Hours:
This is a full-time non-exempt position. Hours will vary according to local school schedules, with typical hours between 7:30 a.m. and 4:00 pm, Monday through Friday.
