Transition Program information

What is a transition program?

Transition programs are structured services designed to help individuals, often students with disabilities or special needs, successfully move from one stage of life or education to another, such as from high school to postsecondary education, employment, or independent living. These programs provide support in areas like job training, life skills, and social development to ensure a smooth and successful transition. They are typically individualized, taking into account each person's strengths, preferences, and goals. Transition programs may be offered by schools, community organizations, or government agencies.

What skills and qualifications are needed to thrive as a transition program coordinator?

To thrive as a Transition Program Coordinator, you need expertise in program management, student counseling, and knowledge of education or social work practices, typically supported by a relevant degree. Familiarity with case management software, educational planning systems, and relevant certifications (such as special education or career counseling) is often required. Exceptional communication, organizational, and problem-solving skills are essential for building relationships and supporting participants through change. These skills ensure effective program delivery, smooth transitions, and positive outcomes for individuals moving between educational or vocational stages.

What are common challenges faced by professionals working in a transition program role, and how can they address them?

Professionals in Transition Program roles often encounter challenges such as coordinating among multiple stakeholders, managing tight timelines, and ensuring effective communication during periods of organizational change. To address these challenges, it is important to develop strong project management and interpersonal skills, maintain regular updates with all involved parties, and stay adaptable to shifting priorities. Leveraging collaboration tools and seeking feedback from team members can also help streamline processes and improve outcomes.

Job description

Where You'll Work
Nurse Residency Program - East Texas area locations including:
  • St. Luke's Memorial Lufkin
  • St. Luke's Memorial Livingston
Job Summary and Responsibilities
Graduate nurses are welcomed from all over the country with the next cohort beginning October 12th, Fall 2026. The Nurse Residency Program (NRP) is designed to support the new graduate nurse during the transition from the student role to the licensed Registered Nurse (RN) role. All St. Luke's Health- Memorial nurse residents receive ongoing professional and educational support from a team of education specialists, nursing leaders, preceptors and mentors. Through in-person and virtually-enhanced education and support, your critical thinking skills, evidence-based decision-making skills, engagement, stress management capabilities, and leadership skills will be developed, nurtured, and optimized. Our residency program accelerates nursing competency by providing in-depth experiences, education, and support during the Nurse Resident's first year. A competency-based curriculum coupled with trained preceptors and active mentorship capitalizes on American Nurses Credentialing Center (ANCC) Accreditation design and innovative, contemporary, and evidence-based practices. Newly licensed registered nurses (RNs) are supported by CommonSpirit's National Nurse Residency Program as they transition to practice at one of our facilities. The program consists of a comprehensive, evidence-based clinical orientation in addition to a 12-month transition-to-practice platform.
Graduate Nurse Residents are responsible for the delivery of excellence in patient care through assessing, formulating nursing diagnoses and establishing goals, planning and implementing intervention, and evaluating patient care outcomes.
At the conclusion of the first year in practice, nurse residents report enhanced self-confidence in providing excellent patient care and function with practice that is in alignment with St. Luke's mission, vision and values. Former nurse residents also serve as mentors and unit leaders, sit on a variety of nursing councils and many are seeking advanced degrees and certifications.
