As a Day Habilitation Program Coordinator Assistant, you'll play a key role in supporting adults in our Day Hab program by promoting independence, dignity, and engaging, person-centered experiences.

What You'll DoSupport Individuals in Everyday Life

Build positive, supportive relationships with clients through active engagement.

Implement individualized program plans, goals, supports, and activities.

Encourage independence, choice-making, and participation in meaningful daily routines.

Support clients with daily living needs, personal care, hygiene, and mealtime assistance when needed.

Enhance Skill Development & Community Involvement

Help clients participate in community activities, outings, and events.

Drive clients using your personal or agency vehicle as needed.

Support clients with money-handling, scheduling, and activity planning.

Ensure Safety, Health & Wellbeing

Follow all safety procedures including proper lifting, universal precautions, and emergency protocols.

Document progress, goals, and daily logs accurately and on time.

Monitor and report health, behavioral, or environmental concerns promptly.

Assist Program Coordination

Support staff training on program plans, documentation, and procedures.

Assist with medication-related duties after completing required training.

Help with audits, recertifications, HCBS requirements, and quality assurance tasks.

Assist with collecting, reviewing, and organizing program data and paperwork.

Support scheduling and coverage needs.
