Job description

Occupational Therapist
We are seeking a compassionate and skilled Occupational Therapist to provide patient-centered rehabilitation services in home, outpatient, assisted living, independent living, board and care, and other community-based settings. In this role, you will help patients improve function, safety, independence, participation, and quality of life through individualized occupational therapy care.
In this role you will collaborate with physicians, providers, rehabilitation professionals, caregivers, and interdisciplinary care team members to support positive outcomes and high-quality care.
Key Responsibilities
  • Perform occupational therapy evaluations and develop individualized, measurable treatment goals.
  • Provide skilled interventions such as therapeutic activities, exercise, neuromuscular reeducation, self-care training, cognitive rehabilitation, caregiver education, and environmental modification.
  • Educate patients and caregivers on adaptive equipment, assistive technology, fall prevention, energy conservation, home programs, and strategies to improve safety and independence.
  • Document evaluations, treatment encounters, progress, reassessments, recertifications, and discharge summaries accurately and timely.
  • Communicate patient progress, barriers, safety concerns, and changes in condition to physicians and care team members.
  • Participate in interdisciplinary care planning, discharge planning, and transition-of-care activities as appropriate.
  • Provide direction and clinical oversight to Occupational Therapy Assistants and support personnel in accordance with applicable regulations and organizational policies.

Qualifications
  • Graduate of an ACOTE-accredited occupational therapy program or equivalent recognized program.
  • Current unrestricted Occupational Therapist license in the State of California.
  • Current CPR/BLS certification.
  • Must maintain required licenses, certifications, credentials, professional liability insurance, and eligibility for Medicare enrollment and payer participation.
  • Minimum one year of clinical experience preferred; home health, outpatient, geriatric, community-based, or rehabilitation experience preferred.
  • Successful completion of required background screening and employment eligibility requirements.
