We are seeking a Family Support Coordinator to serve as a trusted resource for potential donor families during times of crisis, loss, and uncertainty. This role combines family advocacy, emotional support, healthcare collaboration, and donation education to help families navigate end-of-life decisions while honoring the wishes of their loved one.

The Family Support Coordinator serves as a dual advocate, supporting donor families while also helping create opportunities for life-saving and life-enhancing transplantation.

In this role, you will work closely with hospital staff, physicians, HonorBridge clinical teams, and donor families to provide emotional support, education, and guidance throughout the donation process. You will help families understand complex medical situations, navigate the donation journey, and make informed decisions while ensuring they feel respected, supported, and cared for every step of the way.

This position is ideal for professionals with backgrounds in counseling, ministry, social work, psychology, bereavement support, or related fields who are comfortable working independently, building relationships, and supporting individuals during emotionally sensitive situations.

• On-call role requiring 16-hour call shifts
• Must be prepared to respond onsite when on call
• Travel to HonorBridge partner hospitals within and occasionally outside assigned regions is required
• Must maintain the flexibility to support families throughout extended donation processes

In this role, you will:

• Respond to referral notifications and collaborate with the Administrator On Call and clinical teams to assess family support needs.
• Establish relationships with hospital staff, physicians, and healthcare teams to coordinate family support services.
• Review available clinical information and collaborate with donation staff to understand case progression and donation potential.
• Serve as the primary support person for potential donor families throughout the donation process.
• Build trusting relationships with families while providing compassionate emotional support during times of crisis and loss.
• Help families understand complex medical information, including clinical deterioration, brain death testing, and end-of-life processes.
• Facilitate communication between families and healthcare providers to ensure questions are answered and concerns are addressed.
• Advocate for families' needs and support their emotional, spiritual, and informational well-being.
• Collaborate with healthcare teams to determine the appropriate timing and approach for donation discussions.
• Participate in collaborative donation conversations, providing education about donation options and opportunities.
• Present donation information in a compassionate, respectful, and family-centered manner with the goal of achieving informed authorization.
• Support families regardless of their donation decision and ensure they feel respected throughout the process.
• Continue providing care, communication, and support to donor families throughout the authorization and donation journey.
• Connect families with community resources, bereavement support, HonorBridge materials, and Family Services programs as appropriate.
• Maintain accurate documentation and communication related to family interactions and case activity.
• Complete administrative, quality assurance, project-based, and communication responsibilities in a timely manner.
• Participate in ongoing education and professional development related to grief support, family care, donation advocacy, and end-of-life communication.
• Perform other duties as assigned in support of HonorBridge's mission.

We're looking for a compassionate professional who can build trust quickly, communicate effectively in emotionally charged situations, and support families with empathy, dignity, and respect.

• Bachelor's degree required.
• Two or more years of professional experience in counseling, ministry, chaplaincy, social work, psychology, grief support, family services, or a related field.
• Demonstrated ability to support individuals and families during crisis, trauma, loss, or significant life transitions.
• Strong communication, active listening, and relationship-building skills.
• High degree of emotional intelligence, empathy, and compassion.
• Ability to work independently while managing multiple priorities and responsibilities.
• Strong resilience and ability to effectively cope with stress in emotionally demanding situations.
• Understanding of medical terminology or ability to quickly learn and apply healthcare concepts.
• Ability to remain available and provide support throughout extended donation processes.
• Valid driver's license and ability to travel to hospitals throughout the service area.

• Experience working in healthcare, hospice, palliative care, bereavement services, chaplaincy, or hospital-based family support roles.
• Experience facilitating difficult conversations related to serious illness, end-of-life care, or loss.
• Knowledge of organ and tissue donation processes.
• Training in grief counseling, crisis intervention, trauma-informed care, or family systems support.
