About Us

We are a growing home health agency dedicated to providing compassionate, high-quality care to patients throughout the Houston area. We are seeking an organized, detail-oriented Therapy Coordinator to join our team and support our therapy department.

Position Summary

The Therapy Coordinator is responsible for coordinating therapy services, scheduling patient visits, maintaining communication with therapists and physicians, and ensuring timely documentation and exceptional patient care.

Responsibilities
  • Coordinate daily schedules for Physical, Occupational, and Speech Therapists
  • Communicate with clinicians and patients
  • Monitor therapy documentation for accuracy and compliance
  • Assist with patient admissions and scheduling
  • Maintain accurate records within the electronic medical record (EMR)
  • Resolve scheduling conflicts and coordinate staffing needs
  • Provide excellent customer service to patients and healthcare partners
  • Perform other administrative duties as assigned
Qualifications
  • High school diploma or GED required
  • Previous healthcare scheduling or coordination experience preferred
  • Home health experience is a plus
  • Strong organizational and multitasking skills
  • Excellent communication and customer service skills
  • Proficiency with Microsoft Office and EMR systems
