Description:

Role: The Community Health Worker (CHW) located within the Central@29 building and will primarily serve the 11 units focused on the Housing First Integrated Supportive Housing Model. This role will also support the additional tenants that request and/or require support to maintain their housing status.


Scope of Tasks & Responsibilities:

  • Build rapport and connection with clients in an empathetic manner
  • Implement a “global assessment” of basic needs and build a plan prioritized by urgency. 
  • Track documented through care management notes, and in the Homeless Management Information System (HMIS) system. 
  • Focus on the “Social Drivers of Health”: Employment, Utilities, Transportation, Food, Clothing, Healthcare coverage (RHC is an enrollment center for Medicaid, HIP, The Affordable Care Act, Snap, etc.) and Healthcare Homes (consistent health provider).      Assist with connecting individuals to supportive services, based off of need. 
  • Coaching and mentoring on how to initiate, follow up and follow through with any requirements that might be part of the referral process
  • Relationship-building and work with community partners who will be part of the referral and follow up providers for clients.      Developing Community Roundtables will likely reflect this work. This will include community and social service linkage to support the best possible housing retention strategy 
  • The CHW will coordinate with the TCB Property Manager as appropriate and when necessary to assist clients with housing      concerns/challenges. This could include household disputes and conflict resolution, as well as eviction and appeal processes 
  • The CHW will Maintain a high level of confidentiality while interacting with patients and clients with empathy and respect
  • The CHW will advocate for needs of the targeted community and neighborhood 
  • The CHW will uphold RHC’s Mission, Vision, and Values
  • The CHW will treat community members, patients and there staff members with respect and dignity
  • Other duties as assigned
Requirements:

Required Education, Experience and Skill:

  • High school education or GED requested. Experience and skills may substitute for formal education
  • Knowledge of the targeted community, available resources and barriers
  • Able to work with people of diverse socio-economic and cultural backgrounds
  • Effective oral and written communication skills
  • Skilled in group process, problem solving, decision making and case management
  • Respect for cultural and spiritual diversity
  • Transportation with valid Indiana driver’s license and proof of automobile insurance 
  • Personal knowledge of the community and its members have either lived in or around the area (currently or in the recent past)
  • Friendly, outgoing personality
  • Bilingual Preferred but not required
