Shift: Days ( Monday to Friday ) - Four-day work week optional

Job Summary & Responsibilities:

Provide comprehensive primary care to patients of all ages

Diagnose and manage acute and chronic medical conditions

Perform routine exams, preventive screenings, and immunizations

Educate patients on wellness, disease prevention, and treatment plans

Collaborate with care teams to ensure quality, patient-centered care

Maintain accurate, timely documentation in the EHR

Support the mission of serving underserved populations in a community health setting
Education, Licensure, and Experience:
-
Must hold a Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) degree from an accredited medical school
-
Licensed or eligible to obtain medical licensure in the state of Indiana
-
Successful completion of an accredited residency program
-
Board-Certified or Board-Eligible in relevant specialty
-
Dedicated to supporting the mission and values of Indiana Health Centers, Inc.

Preference given to candidates with experience working in Federally Qualified Health Centers (FQHC), Community Health Centers (CHC), or Patient-Centered Medical Homes (PCMH)
