DPI Staffing is seeking a compassionate and dedicated Health/Human Service Case Manager to join our team in Milford, DE. This role is essential in providing personalized support and advocacy to individuals facing personal, social, health, and economic challenges. Apply now!

Case Manager
Location – Milford, DE
Schedule: Full Time 8:00 am to 4:30 pm 
Pay Rate: $19.50 an hour
 
Key Responsibilities:
 
  • Interview clients, families, and others to determine eligibility and connect them with services.
  • Develop and monitor individualized service plans with hospitals, clinics, and community agencies.
  • Provide information and guidance to clients regarding benefits and available resources.
  • Coordinate with agencies to ensure timely service delivery.
  • Maintain accurate case records and documentation.
  • Use computer systems to research and track client data.
Qualifications:
  • 6+ months case management experience.
  • Knowledge of social work practices, Medicaid/eligibility requirements, and community resources.
  • Interview clients, families, and others to determine eligibility and connect them with services.
  • Develop and monitor individualized service plans with hospitals, clinics, and community agencies.
  • Provide information and guidance to clients regarding benefits and available resources.
  • Coordinate with agencies to ensure timely service delivery.
  • Maintain accurate case records and documentation.
  • Use computer systems to research and track client data.
