Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans according to patient needs, goals, and physician direction. This role involves the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate effective return to community activities. The therapist will serve as an educator to patients, families, students, coworkers, and themselves while upholding both organizational values and professional standards.

Responsibilities
  • Complete comprehensive evaluations of assigned patients and develop appropriate treatment plans and goals.
  • Document patient care in a clear, concise, accurate, and timely manner, in compliance with relevant guidelines and organizational policies.
  • Ensure care is delivered under valid physician orders and within insurance approval.
  • Participate in student programs, providing supervision and development of learning objectives.
  • Create and follow an individualized professional development plan, pursuing ongoing education and professional activities.
  • Maintain professional licensure and adhere to the State Practice Act and relevant regulatory standards.
  • Engage in quality improvement initiatives aiming to enhance service delivery.
Required Experience / Certifications / Licensure
  • Degree in Physical Therapy from an accredited school.
  • 1+ year of outpatient clinical experience.
  • CPR Certification.
  • Active and applicable licensure in the state of practice.
  • Previous experience with this agency is highly desired.
