Assisted Living LPN

Shift: 3:00 PM – 11:00 PM
Schedule — Every Other Weekend Plus One Shift Per Week (16 hours)

Shift: 3:00 PM – 11:00 PM
Schedule — Part Time 32 hours a week

Shift: 4PM-8PM

Schedule — Every Other weekend

Position Summary

We are seeking a compassionate, dependable, and experienced Licensed Practical Nurse (LPN) to join our Assisted Living team. The LPN is responsible for providing quality nursing care to residents while ensuring their comfort, safety, and well-being in accordance with physician orders, state regulations, and community policies.

Qualifications
  • Current and active LPN license in good standing
  • Minimum of 3 years of experience as a Licensed Practical Nurse required
  • Previous experience in an assisted living facility preferred
  • Strong clinical, organizational, and communication skills
  • Ability to work collaboratively with residents, families, physicians, and interdisciplinary staff
  • CPR certification preferred

Essential Duties & Responsibilities
  • Administer medications and treatments as prescribed by physicians
  • Monitor residents’ health status and report changes to the appropriate personnel
  • Provide direct nursing care and support to residents as needed
  • Complete accurate and timely documentation in resident records
  • Respond promptly to resident emergencies and medical concerns
  • Supervise and support Certified Nursing Assistants (CNAs) and care staff during the shift
  • Ensure compliance with all state regulations, infection control standards, and facility policies
  • Communicate effectively with residents, families, and healthcare providers
  • Participate in shift report and care planning meetings
  • Promote a positive, respectful, and safe environment for residents and staff

Physical Requirements
  • Ability to stand, walk, bend, and lift as required throughout the shift
  • Ability to assist residents with mobility and emergency situations
  • Ability to work evening hours, weekends, and holidays as schedule
