SCHEDULE DETAILS

Location: This position is located in our Supportive Care and Integrative Oncology department at the Simmons Comprehensive Cancer Center which provides front-line care in the outpatient setting, and opportunities to lead patient programs.

Work Schedule: Monday-Friday, 8:30am-5pm. No on-call. This role offers potential for a hybrid schedule with a mix of onsite and remote work (discuss with manager).

Infrequent participation in early evening or weekend programs; up to 2 times per month subject to department needs with prior notice (Most support groups are during workday; some are evening or weekend monthly options).

With leader approval, this position is eligible to participate in Social Work Clinical Ladder, an "opt in" program that allows for additional compensation upon two years completion of approved activities.
EXPERIENCE AND EDUCATION
Required

  • Education
    Graduate of an Institution certified by the Council of Social Work Education with a Master's degree in Social Work.
  • Licenses and Certifications
    (LMSW) LIC MASTER SOCIAL WORKER by the Texas State Board of Social Work Examiners. Temporary license is acceptable during first 90 days of employment Upon Hire and
    (BLS) BASIC LIFE SUPPORT BCLS is required however, not required in Transplant Departments Upon Hire

JOB DUTIES

  • Participates in multidisciplinary rounds and proactively determines patient needs for discharge transitions including transportation, DME, LTAC, SNF, home health agencies and hospice.
  • Utilizes crisis intervention and problem solving skills to facilitate and enhance patient and family's ability to meet acute and ongoing medical and psychosocial needs.
  • Demonstrates ongoing familiarity with community resources to facilitate positive patient and family outcomes agencies to assure appropriate client access.
  • Maintains effective documentation including tracking and referral information; consistently meets department and UTSW documentation standards.
  • Identifies and reports situations or cases to appropriate governmental agencies as legally required and in compliance with organizational policies and procedures.
  • Serves as an active participant of the health care team, communicating effectively with other health care providers, patients, families, other hospital personnel, and visitors.
  • Identifies the need for professional growth and seeks appropriate development opportunities.
  • Provides education to staff, patients and community resource affiliates.
  • Maintains system for tracking placement of patients after discharge on monthly basis.
  • Has knowledge of Department and Unit Performance Improvement program and is aware of expected participation. Identifies need for professional growth and seeks development opportunities.
