B2 Constructors LLC is seeking a qualified Quality Control Manager / Site Safety & Health Officer (QCM/SSHO) for the Dallas VA Medical Center. This is a dual-hat role required under the VA contract, combining full accountability for the project's Quality Control Plan (QCP) and the Accident Prevention Plan (APP) / Activity Hazard Analyses (AHAs). The QCM/SSHO will be on-site during all active construction activities, working alongside the Project Superintendent, and will serve as the primary point of contact with the VA's Contracting Officer's Representative (COR) on all QC and safety matters.

This position requires a thorough understanding of VA and OSHA construction safety requirements, EM 385-1-1 (or equivalent), VHA facility access protocols, and the three-phase quality control inspection process mandated on federally contracted construction projects.

The QCM/SSHO will provide quality and safety oversight across all work on this civil/site construction project, including:

  • Demolition of existing pavement, curbs, and site improvements
  • Earthwork, grading, cut/fill, and subgrade preparation
  • Storm sewer installation: 18" RCP pipe, inlets, manholes, and tie-ins
  • Erosion and sediment control installation, maintenance, and removal
  • Concrete paving, flatwork, curbs, joints, and ADA-compliant details
  • High-security perimeter fencing and motorized gate systems
  • Pavement markings and site signage
  • Electrical work: demolition, temporary power, underground conductors, and exterior lighting
  • Landscape and irrigation modifications

Quality Control Responsibilities

QC Plan Development & Administration

  • Develop, implement, and maintain the project Quality Control Plan in accordance with VA contract requirements.
  • Establish and maintain definable features of work for each trade and phase of construction
  • Conduct and document all three phases of QC inspections - Preparatory, Initial, and Follow-up - for every DFOW
  • Maintain QC documentation including inspection reports, test logs, non-conformance reports (NCRs), and corrective action records
  • Serve as the primary interface with the VA COR for all QC submittals, inspection notices, and deficiency resolution

Submittals & Materials

  • Manage the submittal log; review submitting applications for specification compliance before forwarding to the VA/A/E for approval
  • Verify that all materials delivered to the site match approved submittals prior to installation
  • Coordinate with the A/E firm on technical questions; note that A/E responses require CO affirmation to be contractually binding
  • Track and resolve all open RFIs in coordination with the Project Manager and Superintendent

Testing & Inspection Coordination

  • Coordinate all construction materials testing: soil compaction, proctor/moisture-density, concrete mix design, compressive strength cylinders, and any electrical testing.
  • Confirm VA-retained testing laboratory schedules align with construction sequencing; ensure retesting of failed areas is assigned and paid per contract
  • Maintain a master testing log with results, pass/fail status, and corrective actions
  • Witness and document key inspections including underground utility installations prior to backfill, concrete placements, and final electrical energization

Closeout & Turnover

  • Compile and organize all closeout documentation: as-built drawings, O&M manuals, warranties, attic stock, equipment schedules, and training records
  • Manage punch list development and tracking through final acceptance by the VA COR
  • Ensure all trade-specific warranties (concrete, markings, planting establishment, fence/gate, electrical equipment) are documented and transferred
  • Coordinate with the Project Manager to prepare the final QC completion report for contract closeout

SSHO Responsibilities

Accident Prevention Plan & Activity Hazard Analyses

  • Develop, submit for VA approval, and implement the project Accident Prevention Plan
  • Prepare Activity Hazard Analyses (AHAs) for each phase of work prior to commencement; review with crews and document acknowledgment
  • Update the APP and AHAs as scope, conditions, or personnel change throughout the project

Daily Safety Management

  • Conduct daily safety inspections of the entire work site; document findings and corrective actions in the daily safety log
  • Lead or co-lead daily pre-task planning meetings and ensure AHAs are reviewed at the start of each new activity
  • Stop work immediately for any imminent danger condition and notify the PM and VA COR; document stop-work events fully
  • Investigate all near-misses, first aid incidents, recordable incidents, and property damage events; prepare written reports and root cause analyses

VA Campus & Facility Safety Requirements

  • Enforce VA Medical Center site access requirements: badging, escort protocols, and restricted area boundaries during all phases of work
  • Coordinate with VA Facility Management for any utility outages, tie-ins, or work within or adjacent to occupied buildings or active roadways
  • Maintain compliant traffic control and inspect and update as work zones shift
  • Ensure all subcontractor personnel have completed required VA site orientation and carry valid site credentials
  • Manage NPDES stormwater compliance: inspect erosion controls after each rain event, document findings, and direct repairs immediately

Subcontractor Safety Oversight

  • Review and approve each subcontractor's safety plan and AHAs before they begin work on-site
  • Conduct weekly safety audits of all active subcontractor operations; document and issue formal deficiency notices as required
  • Maintain OSHA 300 log and all required incident reporting; submit reports to VA within required timeframes

Track subcontractor safety performance metrics; escalate repeat deficiencies to the PM for contractual action

Requirements

Required Qualification

  • Minimum 5 years of experience as a QCM and/or SSHO on federal, VA, DoD, or comparable heavy commercial construction projects
  • USACE Construction Quality Management (CQM) for Contractors certification (current within 5 years) - required for VA federal projects
  • OSHA 30-Hour Construction certification (current); OSHA 510 or equivalent preferred
  • Demonstrated experience developing and implementing Quality Control Plans and Accident Prevention Plans on government contracts
  • Working knowledge of EM 385-1-1 (U.S. Army Corps of Engineers Safety and Health Requirements Manual)
  • Experience conducting three-phase QC inspections and maintaining associated documentation to VA or Corps of Engineers standards
  • Ability to pass VA background investigation and obtain Medical Center facility access credentials
  • Valid driver's license; proficiency with construction management software for document control and reporting

Preferred Qualifications

  • Prior QCM/SSHO experience on VA Medical Center or other active healthcare/federal campus projects
  • Familiarity with VHA Master Specifications and VA Technical Information Library (TIL) requirements
  • CHST (Construction Health and Safety Technician) or CSP (Certified Safety Professional) designation
  • Experience with underground utility and electrical construction QC and safety requirements
  • Working knowledge of NPDES stormwater permitting and SWPPP management on construction sites
  • Experience coordinating with VA Contracting Officers and CORs on submittal review and QC documentation
