Job description

Under general supervision of the Manager of Professional Practice, the Diabetes Educator will act as a liaison to ensure adequate communication between physicians, patients, family/support persons, case managers and outpatient diabetes educational services. The inpatient educator will communicate closely with the outpatient diabetes educators to promote continuum of diabetes education from inpatient to outpatient services. The educator will promote the integration of patient education throughout the Valleywise Health system to meet regulatory and system standards in an efficient, cost effective manner. The educator serves as a resource to patients, staff and the community in diabetes related topics.
Annual Salary Range: $76,835.20 - $122,928.00
Qualifications
Education:
  • Requires a Bachelor's degree in Nursing or Nutrition; or an equivalent combination of training and progressively responsible experience that will result in the required specialized knowledge and abilities to perform the assigned work.

Experience:
  • Requires a minimum of three (3) years' experience in nursing or as registered dietitian, preferably with prior experience as a Diabetes Educator.

Certification/Licensure:
  • Must possess a current, valid AZ RN license; temporary AZ RN license or valid compact RN licensure for current state of practice; or Registered Dietitian (RD) certification/licensure.
  • Must also be in good standing with the issuing Board of Nursing or issuing authority for Registered Dietitian certification.
  • Requires CDCES (Certified Diabetes Care and Education Specialist) certification within 18 months of hire.

Knowledge, Skills, and Abilities:
  • Requires clinical expertise and advanced knowledge of Diabetes Education.
  • Requires strong leadership mentoring, communication, and organizational skills.
  • Must demonstrate effective problem solving, prioritization, and decision-making skills.
  • Requires facilitation skills, ability to handle multiple complex projects, excellent written and verbal communication skills, time management skills, leadership skills, critical thinking, and SBAR communication model.
  • Must be able to model professional behavior.
  • Ability to work with culturally diverse people is a must.
  • Requires the ability to read, write and speak effectively in English.
