Job description

Position Title
Telehealth Outreach Coordinator
Care Collaborative Hays
Position Summary / Career Interest:
The Telehealth Outreach Coordinator is responsible for creating a seamless patient experience by coordinating and scheduling appointments for patients. The Coordinator supports on-site and virtual patient encounters including implementation and management of telehealth equipment and resources, including travel to rural health clinics. Duties include patient scheduling, registration, transportation of outreach providers, rooming of patients and grant report generation.
The Telehealth Outreach Coordinator monitors and responds to appointment requests within the electronic medical record (EMR) workqueues, while also working as a part of the clinical team to answer incoming calls and messages from both clinicians and patients.
Additionally the position supports staff with coordination of patient navigation and health coaching services offered within the department and performs clerical duties independently, including but not limited to answering phones, maintaining fax queue, correspondence, filing, copying, scanning, maintaining travel/outreach calendars and meeting schedules.
Responsibilities and Essential Job Functions
  • Coordinates clinic times and locations
  • Manages the transportation of healthcare team
  • Responsible for scheduling, registering, and collecting relevant paperwork for billing of patients
  • Conducts regular travel to rural health clinic locations, as scheduled approximately 4 times per month with potential to increase as program grows
  • Rooms patients, collects medical history and obtains vital signs for patient to provider
  • Manages the scheduling of required testing
  • Supports on-site UKHS outreach physicians
  • Generates grant-related reports
  • Works closely with the Telehealth Program Manager to assure compliance with all federal program requirements
  • Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.

Required Education and Experience
  • 3 or more years administrative assistant or comparable experience

Preferred Education and Experience
  • Bachelors Degree from an accredited College or University.

Required Licensure and Certification
  • Valid Kansas Driver's License
  • Certified Nursing Assistant or Certified Medical Assistant.

Knowledge Requirements
  • Technical skills including telehealth tablets, broadband connectivity and electronic health record utilization.
  • Additional certification may be required for certain patient care settings or populations.
