Responsibilities
  • Conduct comprehensive psychosocial assessments to identify patients' emotional, social, behavioral, and environmental needs
  • Develop individualized treatment and care plans based on patient assessments
  • Provide individual, family, and group counseling to support emotional well-being and recovery
  • Offer crisis intervention and emotional support for patients and their families
  • Coordinate discharge planning and connect patients with appropriate community resources and support services
  • Collaborate with physicians, nurses, case managers, therapists, and interdisciplinary healthcare professionals to ensure coordinated patient care
  • Advocate for patients to ensure access to healthcare services and community programs
  • Educate patients and families regarding available resources, coping strategies, and ongoing support services
  • Maintain accurate, timely, and confidential documentation in the electronic medical record (EMR)
  • Participate in multidisciplinary care meetings and contribute to patient care planning
  • Ensure compliance with facility policies, regulatory standards, and ethical social work practices
Requirements
  • Active Licensed Clinical Social Worker (LCSW) License in North Dakota (or eligibility for licensure)
  • Minimum 1 year of recent clinical social work experience
  • Master's Degree in Social Work (MSW) from an accredited institution
  • Current BLS Certification preferred
  • Experience in hospital, acute care, behavioral health, or outpatient settings preferred
  • Strong assessment, counseling, crisis intervention, and case management skills
  • Excellent communication, interpersonal, and organizational abilities
  • Experience using electronic medical record (EMR) systems preferred
  • Ability to work independently while collaborating effectively with multidisciplinary teams
  • Commitment to providing compassionate, patient-centered care
