  • Performing psychiatric evaluations, SBIRT screenings, diagnostic assessments, and applying DSM-5 criteria to support accurate diagnosis and treatment planning
  • Monitoring patient progress, evaluating treatment effectiveness, and adjusting care plans based on clinical outcomes and evolving patient needs
  • Collaborating closely with physicians, advanced practice providers, nurses, care managers, and community organizations to coordinate comprehensive, patient-centered care
  • Educating patients and their families on treatment plans, mental health conditions, medications, coping strategies, symptom management, and available community resources
  • Maintaining timely, accurate documentation including assessments, progress notes, treatment plans, service coding, and clinical communications within the electronic medical record (EMR)
  • Ensuring patient confidentiality and safeguarding medical records in accordance with HIPAA regulations and professional standards
  • Supporting appropriate levels of behavioral health care through ongoing assessment, coordination, and referral based on patient acuity and clinical needs
  • Pursuing continuing education and professional development opportunities to strengthen clinical expertise and remain current with evidence-based behavioral health practices
This role might be for you if:

  • You are highly organized and committed to maintaining accurate clinical documentation and regulatory compliance
  • Adapting your therapeutic approach to meet the unique needs of diverse patient populations motivates you
  • Continuous learning and professional development are important to your long-term career growth

To be considered for this Licensed Clinical Social Worker opportunity, you are passionate about providing compassionate, evidence-based behavioral healthcare while supporting patients and families through every stage of their care journey.

This position is a full-time outpatient opportunity with a schedule based on departmental needs. The primary shift is a full-time day shift.
