Job Summary:

The Outreach & Engagement Specialist is responsible for outreaching to prospective clients and their families to screen and introduce the ECM benefit available to support a whole person care approach to improving health outcomes and addressing social determinants of health.ย  The goal is to demonstrate to prospective members the benefit of enrolling in ECM in order receive assistance in coordinating all primary, acute, behavioral, developmental, oral, social needs and long-term services and supports for members wellbeing.

The Outreach & Engagement Specialist is an energetic, outgoing self-starter and able to work independently. Relationship building is a key skill for the Outreach & Engagement Specialist. The Outreach & Engagement Specialist must be skilled in developing a rapid rapport with potential clients. They must be flexible, possess excellent organizational and time management skills. The Outreach & Engagement Specialist must operate from a strengths-based and โ€œwhatever it takesโ€ perspective. This role requires significant flexibility and must manage an array of duties and assignments for the ECM program.

Requirements:

  • Minimum of high school diploma or GED.
  • Minimum of 6 months related experience and/or training working with underserved or difficult to reach populations.
  • Must have lived experience of navigating systems of care (some examples include: medical; housing; mental health, substance use, regional center, education etc.) for self or a loved one.
  • Bilingual Spanish required (verbal and written skillset).
  • Valid California driverโ€™s license; current automobile insurance; have and maintain a clean driving record acceptable to the organizationโ€™s insurance company; have immediate access to his or her vehicle during work hours.

Position Location: Commerce, CA

Salary Range: $20.09 up to $29.36 per hour

Additional $2.00 per hour bilingual Spanish differential.

Hybrid work schedule and service delivery via telehealth available.

Flexible schedules are available upon successful completion of introductory period.

Eligible employees may request up to 5 days of Paid Time Off to study for their licensure.ย
