Services SFT est a la recherche d'un(e) Travailleur(se) social(e) au sein du reseau de la sante sur la magnifique Cote Nord. Vous aurez l'occasion de travailler aupres d'une clientele variee dans un cadre stimulant.

Responsabilites principales :

  • Evaluer la situation psychosociale des usagers et de leur famille.
  • Intervenir aupres de clienteles en difficulte (sante mentale, dependance, jeunesse, personnes agees, etc.).
  • Elaborer et mettre en uvre des plans d'intervention individualises.
  • Assurer un accompagnement et un soutien aux usagers dans leur parcours de soins.

Exigences :

  • Baccalaureat en travail social ou en service social.
  • Membre en regle de l'Ordre des travailleurs sociaux et des therapeutes conjugaux et familiaux du Quebec (OTSTCFQ).
  • Experience recente et pertinente dans le secteur en DPJ
  • Experience recente et pertinente en jeunesse et service de proximite
  • Excellente capacite d'ecoute, d'adaptation et de travail en equipe.
  • Disponibilite a travailler a temps complet pour toute la duree du mandat.
  • Etre Bilingue
