Licensed Chaplain (Home Health & Hospice)Now Hiring – Licensed Chaplain (Full-Time, Part-Time & PRN)

Locations: North &  South  Austin, TX  
Elite Home Health & Hospice is seeking a compassionate and dedicated Licensed Chaplain to provide spiritual and emotional support to patients and their families receiving Home Health and Hospice services. The Chaplain is an integral member of the interdisciplinary team and provides spiritual care that respects the patient's beliefs, values, culture, and personal preferences.

Position Summary
The Licensed Chaplain provides spiritual counseling, emotional support, crisis intervention, grief support, and assists patients and families in coping with illness, loss, and end-of-life issues. The Chaplain works collaboratively with physicians, nurses, social workers, and other healthcare professionals to provide holistic, patient-centered care.
Responsibilities
  • Provide spiritual care visits to Home Health and Hospice patients and their families.
  • Perform comprehensive spiritual assessments and develop individualized plans of care.
  • Provide emotional and spiritual counseling to patients, families, and caregivers.
  • Participate in Interdisciplinary Group (IDG) meetings and collaborate with the healthcare team.
  • Provide grief support and bereavement counseling when appropriate.
  • Assist patients with advance care planning and end-of-life concerns.
  • Coordinate spiritual resources with community clergy when requested.
  • Maintain timely and accurate documentation in accordance with agency policies and regulatory requirements.
  • Respect and support diverse religious, cultural, and spiritual beliefs.
  • Provide crisis intervention and emotional support during difficult circumstances.
  • Participate in orientation, annual competency evaluations, and continuing education programs.
Qualifications
Required Qualifications
  • Master's Degree in Divinity (M.Div.) or Master's Degree in Theology (M.Th.).
  • Licensed or Ordained Minister, Pastor, Chaplain, or Clergy Member in good standing.
  • Minimum one (1) year of pastoral ministry, chaplaincy, hospice, healthcare, or counseling experience preferred.
  • Excellent verbal and written communication skills.
  • Ability to provide compassionate, patient-centered spiritual care.
  • Valid Texas Driver's License.
  • Reliable transportation and automobile insurance.
  • Ability to travel within assigned service areas.
Preferred Qualifications
  • Hospice or Home Health experience preferred.
  • Clinical Pastoral Education (CPE) training is preferred but not required.
  • Experience providing grief counseling and bereavement support.
  • Knowledge of Medicare Hospice regulations and interdisciplinary team practices.
  • Bilingual candidates are encouraged to apply.
