Remote Nurse Practitioner – TCM / CCM

Schedule: Full-Time | Monday–Friday
Salary: $120,000 Annually
License Requirement: Active New York State Nurse Practitioner License REQUIRED

MedStaff Group is recruiting for an experienced, dependable, and clinically strong Nurse Practitioner who is actively licensed in New York State for a full-time, fully remote position focused on Transitional Care Management (TCM) and Chronic Care Management (CCM).

Candidates who are not currently licensed as Nurse Practitioners in New York State will not be considered.

Position Overview

The Nurse Practitioner will manage a full caseload of patients requiring transitional and chronic care management. This position requires a provider who is organized, proactive, clinically confident, and capable of independently managing a high-volume patient population in a remote environment.

The provider will take ownership of assigned patients, ensuring timely follow-up, appropriate clinical intervention, accurate documentation, and strong coordination across the entire care team.

Responsibilities
  • Manage a full caseload of TCM and CCM patients
  • Conduct remote patient assessments, follow-up visits, and ongoing clinical reviews
  • Manage patients with multiple chronic medical conditions
  • Review hospital, skilled nursing facility, and other facility discharge information
  • Complete appropriate post-discharge follow-up within required timeframes
  • Perform medication reconciliation and review treatment plans
  • Identify changes in patient condition and provide appropriate clinical intervention
  • Coordinate care with physicians, specialists, facilities, pharmacies, patients, and families
  • Review laboratory results, symptoms, medications, and other relevant clinical information
  • Provide treatment recommendations and clinical guidance within scope of practice
  • Ensure TCM and CCM requirements, outreach, assessments, and follow-ups are completed on time
  • Maintain accurate, detailed, and timely clinical documentation
  • Track outstanding patient needs and follow through until resolution
  • Communicate significant clinical concerns and care plan updates appropriately
  • Take full ownership of assigned patients and continuity of care
Requirements
  • MUST hold an active and unrestricted New York State Nurse Practitioner license
  • New York State NP licensure is required at the time of application
  • Active DEA registration
  • Active enrollment/credentials with Medicare and Medicaid
  • Comfortable independently managing a full patient caseload
  • Strong clinical judgment and independent decision-making ability
  • Excellent communication, organization, and documentation skills
  • Strong time-management and follow-up skills
  • Proactive, dependable, and responsive
  • Comfortable working independently in a fully remote environment
  • Previous experience in TCM, CCM, primary care, chronic care management, care coordination, skilled nursing, or transitional care strongly preferred
Compensation & Schedule
  • $120,000 annual salary
