Starting Pay: $28.00/hour


Family Creations is a nationally recognized surrogacy and egg donation agency that has spent more than 20 years helping build families through third-party reproduction. We are seeking a compassionate, organized, and highly motivated professional to join our team as a Surrogate Recruitment & Intake Coordinator with defined opportunity for growth into Director of the intake department after training and growth.


This position combines relationship-building, recruiting, customer service, and administrative coordination. You will serve as one of the first points of contact for women interested in becoming surrogates, guiding them through the initial stages of the screening process while ensuring every detail is documented accurately and efficiently.


While this position is primarily remote, candidates should be located within driving distance of the Des Moines metro area for in-person training and occasional meetings at our West Des Moines office.


What You'll Do


  • Collect, organize, and track required documentation for surrogate candidates throughout the screening process
  • Maintain detailed and accurate records to ensure each candidate's file is complete and up to date
  • Follow up with candidates regarding missing documents, forms, and outstanding requirements
  • Monitor candidate progress through multiple screening stages and ensure deadlines and next steps are completed in a timely manner
  • Serve as a primary point of contact for prospective surrogates during the intake and screening process
  • Conduct phone and Zoom interviews with prospective surrogates
  • Educate candidates about the surrogacy process and agency requirements
  • Build positive relationships with applicants and provide ongoing support and communication
  • Coordinate with records, matching, and case management teams to facilitate a smooth screening process
  • Assist with creating and updating surrogate profiles
  • Identify potential concerns, discrepancies, or missing information and escalate when appropriate
  • Support recruitment initiatives and special projects as needed
