What You'll Do

Essential Responsibilities:

  1. Provide services in the community where the consumer may need it, this could include but is not limited to school, work, home, shelter, store, etc.
  2. Provide outreach, support, referral to services and connection of resources to the consumer and or their family.
  3. Provide ongoing strengths assessment eliciting the consumer and or their family as active participant(s) in determining strengths, wants, needs and preferences.
  4. With input from the client, their family and other members of the treatment team develop and document treatment goals and plan related services and activities. Monitor and document progress and concerns adjusting the person-centered plan as needed.
  5. Advocate for client rights and community integration. Preserve dignity and confidentiality of each person on caseload.
  6. Responsible for the implementation of scheduled day activities such as exercise, socialization, personal goal setting, illness management and community outings as appropriate.
  7. Provide individual therapeutic interventions to assist in assessment of symptom management and skill development.
  8. Review client incidents with supervisor and submit written report as outlined in policy and procedure.
  9. Within scope of knowledge, provide resources and education to the client and their family about mental illness or serious emotional disturbances upon request.
  10. Meet administrative and clinical documentation requirements both in terms of content and timeliness as outlined in agency policy.
  11. Meet client care expectations in terms and comply with other established productivity standards as established.
  12. Attend and actively participate in clinical supervision and program staffing. Provide input to supervisors and other treatment team members regarding clinical and program issues.
  13. The provision of crisis management and/or coordination of emergency services during periods of crisis.
  14. Drive to community settings for the delivery of services. This includes transporting the client in a personal or company vehicle.
  15. Enter and maintain clinical schedule in the agency electronic health record.
  16. Maintain and advance knowledge and skills related specifically to position or to field of work through regular review of pertinent materials or participation in training events as approved or directed by supervisor.
  17. Adhere to the standards, policies and instructions established by PMHS Policy and Procedure and any applicable ethical codes within field of employment.
  18. Demonstrate sensitivity to all cultural and ethnic differences.
  19. Review and comply with the Pawnee Code of Conduct outlined in the Employee Handbook.
  20. Complete a state required training specific to the focus of the position. This is required at the first available opportunity.
  21. Work cooperatively with the Management Team and other members of the staff in sharing resources and expertise in pursuit of the agency's mission.
  22. Perform all other reasonably related duties as assigned by immediate supervisor or other management personnel.
Requirements:

Required:

  • Must be at least 21 years old.
  • Bachelor’s degree in a human service field or equivalent work experience, or a combination of work experience in the human services field and education with one year of experience substituting for one year of education.
  • Strong oral and written communication skills using the English language.
  • A valid driver’s license and automobile liability insurance maintained in accordance with Pawnee policy and procedure.
  • Basic personal computer skills which enable the entry, retrieval and use of electronic data.


Preferred:

  • Experience with Microsoft Office software preferred.
  • Some collegiate-level schooling in a human services field.


All employees will be required to submit to and pass a background check and drug screening.
