Reporting to an Outreach Coordinator, this essential-staff role provides direct and indirect care that promotes independence, health, safety, and meaningful community participation. What You Will Do:
โ€ข Assist with and monitor activities of daily living while encouraging independence, competency, choice, and decision-making.
โ€ข Promote quality of life through empowerment, safety awareness, dignity, respect, and protection of individual rights.
โ€ข Monitor overall health and well-being and support medication treatment and administration in accordance with Medication Administration Program (MAP) requirements.
โ€ข Provide calm, responsive crisis intervention and emergency support as needed, following program and agency procedures.
โ€ข Help individuals maintain and expand their social-support networks, family and guardian relationships, community connections, and participation in meaningful activities.
โ€ข Plan community activities and provide safe, timely transportation for appointments, recreation, shopping, errands, and travel between individual residences or program locations.
โ€ข Implement assigned strategies that support Individual Service Plan and Person-Centered Treatment Plan (ISP/PSTP) goals and objectives.
โ€ข Complete progress notes, health and medication records, incident reports, medication occurrence reports, program forms, and other time-sensitive documentation accurately and on time.
โ€ข Prepare balanced, nutritious meals and snacks and complete routine cleaning, household maintenance, and assigned program responsibilities, with individuals' participation whenever appropriate.
โ€ข Recognize and promptly report health, safety, medication, maintenance, and other program concerns to the appropriate supervisor.
โ€ข Attend required meetings, in-services, and trainings; maintain all mandatory training requirements and apply feedback and new learning in daily practice.
โ€ข During assigned awake overnight shifts, contact the supervisor at scheduled hourly intervals and report the status of individuals served and completion of assigned tasks.
Who You Are:
โ€ข High School Diploma or GED required.
โ€ข Six months of human-services experience preferred; BHN provides training.
โ€ข Valid driver's license and a safe, reliable personal vehicle available during all working hours required.

โ€ข Strong interpersonal, organizational, time-management, and problem-solving skills, with the ability to manage multiple responsibilities and respond effectively to urgent needs.
