Job description

Core Function:
Under the direction of the Program Coordinator, participates in the planning of appropriate program activities and providing high quality care and support to clients of the Community Care and Wellness Hub, in accordance with the organization's relationship-centered care approach, relevant government standards, client's rights and the organization's mission, vision and philosophy.


Qualifications:
1. Post-Secondary training in a relevant health (e.g., therapeutic recreation, kinesiology, nursing) or social services (e.g., social service work, community services, gerontology) discipline or equivalent; Completion of an approved Personal Support Worker (PSW) course in Ontario and/or a certificate in geriatric care, an asset.
2. Experience in community services within a geriatric care setting, specifically providing direct care and support to individuals with complex needs
3. Completion of a current C.P.R.
4. Good interpersonal and relationship-building skills
5. Satisfactory Police Reference Check (with vulnerable sector screen) result
6. Good oral and written communication skills in English and Chinese
7. Competency in standard office software and digital record-keeping


Responsibilities:
1. Under the overall direction of the Program Coordinator, participates in the planning and delivery of holistic, relationship-centered program services.
2. Implements and ensures the smooth operation of program activities within a safe, hygienic, and supportive environment.
3. Provides high-quality care and support to assist clients in their daily activities and personal hygiene, while respecting dignity, rights, and privacy.
4. Participates in huddles and debriefing sessions to review and update of client care plans, providing frontline observations and input on client progress and care needs.
5. Supports the facilitation of daily program activities and social-recreational sessions by providing individual support and encouraging client engagement.
6. Collaborates with the team to distribute, portion, and serve meals, providing meal assistance in accordance with the dietary needs and preferences outlined in the client's care plan.
7. Assists the team in preparing program materials and setting up the physical environment for daily operations.
8. Responds promptly to client's care needs including medical emergencies or behavioral incidents, taking appropriate action following the organization protocols.
9. Assists clients in the proper use and care of adaptive devices to promote independence and physical well-being.
10. Complies with all risk management policies, including health and safety standards and infection control protocols.
11. Maintains accurate records of daily client care, participation, and observations as required.
12. Other duties as assigned.
