Familiarity with client management systems, the NDIS portal, and documentation standards is typically required. Support coordination is a service that helps individuals with disabilities or complex needs to access and manage the supports and services they require. A support coordinator works with clients to develop a plan, connect them with relevant providers, and ensure that their needs are being met efficiently. The overall goal is to build the individual's capacity and independence while making sure their supports are well-coordinated.
