Key Responsibilities of the LPN / LVN
  • Collects data relevant to resident's physical and psychosocial condition on an ongoing basis and communicates findings to RN and other members of the Interdisciplinary Team.
  • Contributes to the creation of a developmentally age-appropriate and culturally relevant plan of resident care reflecting nursing diagnosis and interventions.
  • Recommends revisions to the plan of care after evaluation of resident outcomes.
  • Collaborates with the resident/family and members of the Interdisciplinary Team ensuring customer satisfaction.
  • Maintains current knowledge and competency in nursing practice and participates in mentoring/orientation of staff.
Requirements of the LPN / LVN
  • NY LPN Licensure
  • BLS (AHA or RQI specifically)
  • 1 year experience as an LPN, preferably in a long-term care facility
