Job Description
Job Title: LPN
Education: LPN license (TN, AR, or MS)
Experience: 3 years clinical (preferably hospital setting)
Summary: The Licensed Practical Nurse will maintain a current license in the States of Tennessee, Arkansas, or Mississippi, and must have three (3) years of clinical nursing experience. Under the supervision of the RN Area Manager, the LPN will assist in the assessment, planning, implementation, and evaluation of client care.
Hourly Pay: $24.00

Shift: Full-Time; Monday - Friday 7a-3p, 3p-11p & 11p-7a

Essential Job Functions:
  • Performs monthly and quarterly assessments as scheduled. PRN assessment of changes in client’s health status and documents findings according to SRVS policies and procedures.
  • Assists in the maintenance of client medical records to assure complete, accurate, and pertinent information on all health conditions, safeguard the security, accessibility and organization according to SRVS policy.
  • Assure prevention and/or early identification of client health needs by making frequent client rounds, assess health conditions by taking, recording, and reporting vital signs and other clinical observations as appropriate.
  • Completes daily shift duties according to SRVS policies and procedures, maintains transfer packets, performs nail care, and maintains accurate menses and seizure records, intake and output records, and any other health tracking forms as needed.
  • Assists in maintaining adequate drug and pharmaceutical stocks to provide continuity and accuracy in ordering and receiving medications form pharmacy.
  • Administers medication and treatments as ordered using the “7 rights” of medication administration (right medication, right dosage, right route, right individual, right time, right texture, and right position).
  • Maintains harmonious work relationships with members of the interdisciplinary team and peers.

Non-Essential Job Functions:
  • Participate in and/or conduct health training for facility personnel as directed by Nurse Manager.
  • Participates in the communication process both verbally and in writing of documents pertaining to client health monitoring and health care prevention.
  • Provides assistance to RN Area Manager on matters directly related to the well-being of the individual.
  • Reviews all consults, recommendations, and MD orders and transcribes orders appropriately.
  • Attends and participates in a professional manner for at least 80% of all scheduled meetings and 100% of all mandatory meetings.

Must be 18 years or older. Must have a valid TN, AR, or MS drivers' license.  Must be able to pass all required pre-employment screenings, criminal background checks, and a drug test.
