Job description

We are seeking a dedicated and experienced Registered Nurse (RN) and/or Licensed Practical Nurse (LPN) to join our team as a Charge Nurse. In this role, you will support the Subacute Unit Manager in driving positive patient outcomes and delivering high-quality care.
Must have Subacute Experience!

Key Responsibilities:

  • Assist with oversight of patient care and outcomes
  • Provide support with admissions and discharges
  • Perform and manage wound care
  • Monitor and support cardiac patients
  • Communicate effectively with patients and families, including outreach as needed

Qualifications:
  • Prior experience in subacute or skilled nursing preferred
  • Strong clinical skills in wound care and cardiac care
  • Experience with admissions, discharges, and care coordination
